Asthma
Parent/Carers of children diagnosed with Asthma will be required to provide the completed Asthma Medical Management Plan upon Enrolment.
Children are unable to attend our Qb services without this information.
Any Ventolin/Asmol required is to be provided to the service, labelled and in a plastic container. If your child requires a spacer this is also to be provided labelled and included in the plastic container.
Educators at your child's service will complete a risk minimization plan in consultation with you.
